Please check one below and return this form with your payment to DMG. 1f a CD is requested and the permit is too large for a CD, you will be called. Please allow up to 5 business days from the receipt of this signed document for your CD or DVD to be available. CD Qa $5.00 per disk _DVD @ $20.00 per disk Please check if you wan[ a copy of the Quick View installation instructions. Computer requirements to use the DMG document imaging CD (or DVD): 1. A program called QuickView will be copied to the CD and must be installed on your computer in order to use a DMG imaging CD the first time. Future use of this or other imaging CD's does not require a new installation of the software. 2. The CD or DVD that you purchase must be in the D: drive. A DVD drive will read a CD or DVD. 3. The CD will contain files with the .tif extension. There needs to be an image viewing application Cor .tif files on your computer that will handle single and multi-page documents. On a Windows XP computer, the default viewing programs arc called "Windows Picture and Fax V fewer" or "Paint" and they do not work with QuickView. The Imaging Viewer that comes with pre-Windows XP computers does work. DMG has determined that a program called "TrfanView",which can be downloaded from IrfanView.com, does work with QuickView. After downloading'9rfanView", ii needs to be configured as your default .tif viewer. DMG does not support or endorse this software and the licensing and registration are the user's responsibility. There are many other programs that work as well, such as "Imaging Preview" and "Imaging for Windows". Instructions• 1. ]nsert the CD into your computer. 2. Allow, time for the CD to install. 3. Eject the CD from the computer, 4. Reinsert [he CD into the computer. 5. You are ready to view [he Permit Files. NOTE: A program called QuickView Automn, which runs the QuickView setup program, has also been copied to this CD. if the Autorun program does NOT work, you will need to manually install the QuickView program. instructions far manually installing QuickView are available upon regttest with the CD or can be viewed at www.minine.state.co.us. The Permit Flle document images can he viewed in Ute reception area of the DMC offiee or they can he purchased on o CD or DVD. The CD/DVD purchase cost covers the time and labor of making the CD. DMG cannot provide computer related suooort.
