The inspection was conducted on January 10 and 11, 1991 followed <br />by the final report dated February l9, 1991. <br />As I mentioned by telephone today, while we believe we provided you with a copy of the <br />original report, we cannot recall with certainty whether a copy of the report was actually forwarded to <br />you. As such, you accepted our offer to provide you with a copy of the report. Please find enclosed a <br />copy of the report for your records. Please feel free to contact us if you have any questions concerning <br />the contents of the report. <br />Please be aware that if additions or modifications have been made to your dwellings or <br />structures since the time of the original inspection, you may request from the Colorado Division of <br />Minerals and Geology (CDMG) an additional pre-blast survey of such additions or modifications. <br />Please contact the CDMG at the following address: 1313 Sherman Street, Room 215, Denver, <br />Colorado 80202, telephone number (303) 866-3567 to make such a request. CDMG will then request <br />that Colowyo promptly conduct the evaluation and provide a copy of the report to both you and <br />CDMG. <br />Please also be aware that if you disagree with the results of any surveys you may notify, in <br />writing, both Colowyo and CDMG of your specific azeas of disagreement, <br />Please contact me should you have any questions concerning this information.
