Long <br />Additional opMpD~ree~o, <br />I would like to follow up our phone conversation of today with this <br />written summary of what additional documents are needed by this <br />Division concerning the above-captioned subject. Materials you <br />have provided to-date are helpful. The following information and <br />commitments from you are necessary at this point: <br />1. Mining Plan. All modifications need to be defined regarding <br />specific portions of the permit area, activities that have <br />been done by the permittee, activities that will now not be <br />done, and proposed activities not yet part of the permitted <br />plan. The mill site must be discussed. <br />2. Reclamation Plan. The items listed in ,#1 also apply here. <br />All shaping, earthwork, piping, closure, and structures must <br />be discussed. <br />3. Environmental Protection. The items listed in 81 and #2 must <br />include information specific to existing and future <br />environmental quality. This will include nature of discharge <br />water (how it enters the mine and what it encounters), how <br />discharge water is to be handled, and why you propose that <br />method (such as plugging adit, onsite treatment, evaporation <br />pond, diversion piping, etc.), nature and contents of waste <br />rock, recent and historic structures which stay or remain or <br />must be built (and how that may impact the site), and <br />stormwater runoff protection. <br />4. Please indicate whether the Colorado Department of Health has <br />determined that you need a discharge permit or stormwater <br />permit, since this will affect your reclamation. Discuss any <br />pertinent practices or structures. <br />5. Water samples. Please provide evidence of recent water sample <br />collection and preservation methods, plus testing methods used <br />at the lab. <br />
