Please add a date of submittal for Annual <br />Hydrologic Reports (suggest as a Last gazagraph on page 4.05-15). <br />Rule 4.06.1(2) <br />Item 59 Page 4.06-1 line 21. Please change so much of this sentence that reads "requested" <br />to read "approved". <br />-Rule-4.07.-3 and-Ru1e4:13 - - - - - - - - - - - <br />Item 60 Page 2.05-581ines 15 through 17; page 4.07-1 line 19. If the collapsed piezometer wells <br />(Streeter Fill) have been abandoned, permanent sealing is required, followed by submission of <br />abandonment reports. Use for future monitoring is unclear (page 4.09-2). Please advise the <br />Division of the status of this reclamation or continued use of the collapsed piezometer wells. <br />Item 61 Drill holes and wells aze developed for a number of reasons, and reports aze required for <br />their development, completion and abandonment. These reports have been scattered throughout <br />the PAP (examples are Exhibit 7 Section 7for Well A6, Exhibit 12 for a Streeter Fill Monitoring <br />Well). To assist in development of reclamation cost estimates, and to acquire documentation for <br />bond release, the Division suggests that an Exhibit be developed as a repository for such reports, <br />or as a minimum, a reference to the location for such reports, for drill holes and wells that require <br />reclamation within the Permit azea. These wells aze summarized on page D 34 in Section D of <br />Exhibit 13. Please add Well 61171 to this reclamation cost estimate. Exploration holes <br />authorized by Exploration Permits should not be included. <br />Rule 4.11.4(2) <br />Item 62 Page 4.11-1. Please add that the waste is placed above the water table; i.e., change <br />the last sentence to read "No waste is allowed on the bottom of the baclcfdl area or below <br />the anticipated groundwater level of backfilled areas." <br />Item 63 Page 4.11-1. Please add to this section, with across-reference to the reclamation <br />plan, that portion of a pit reserved for disposal of facility demolition materials at the time <br />of mine closure and reclamation. <br />Item 64 Page 4.11-1. Please add to the PAP a reference to County approval of in-pit disposal <br />of inert, non-toxic demolition materials and operating trash. Please include that demolition <br />materials of sedimentation ponds are to be place in the ponds during reclamation or are to <br />be transported to the pit area. <br />Rule 4.18 <br />Item 65 Exhibit 11.
