the attachments for equivalent "quick-disconnect" pressure gauges certified <br />for ninety-five (95) percent accuracy, or better, throughout the range of <br />permitted operation in order to verify values for injection pressure being <br />recorded from the continuous monitoring device. <br />(b) Wellhead pressure of the tubing/casing annular space: a continuous <br />pressure monitoring device in the wellhead casing/tubing annulus shall be <br />connected to a device with a sampling frequency of at least every 15 seconds <br />and aone-half (1/2) inch Female Iron Pipe (FIP) fitting, isolated by plug or <br />globe valves, and located on the tubing/casing annulus; and the above <br />fittings shall be positioned to allow attachment of one-half (1/2) inch Male <br />Iron Pipe (MIP) pressure gauges or the attachments for equivalent "quick- <br />disconnect" pressure gauges certified for ninety-five (95) percent accuracy, <br />or better, throughout the range of permitted operation in order to monitor the <br />annulus fluid pressures. <br />The tubing/casing annulus shall be maintained full of either fresh water <br />treated with anon-toxic corrosion inhibitor or other packer fluid as <br />approved, in writing, by the Director. This fluid shall be maintained under <br />a positive pressure of between 20 and 80 psi using a nitrogen gas blanket. <br />A diesel freeze blanket or other fluid as approved, in writing, by the Director <br />maybe circulated from surface to below frost level at completion to prevent <br />freezing and possible equipment failure during winter months. <br />(c) Automatic well shutdown: the continuous monitoring system shall have an <br />automatic well shut down switch installed which shall automatically shut-in <br />the well if either of the following occur: <br />(i) The surface injection (tubing) pressure shall be maintained between <br />200 to 930 psi. Any operation outside of this range shall result in an <br />immediate shut down of the injection pumps; <br />(ii) Because the gas pressure will vary as a result of fluctuation in the <br />injectate temperature, the tubing/casing annulus pressure shall be <br />maintained between 20 and 80 psi. Any operation outside of this <br />range shall result in an immediate shut down ofthe injection pumps. <br />When adjusting the gas pressure, the operator shall use the target <br />value of 50 psi; or <br />(iii) If there is a 100 psi or greater drop in injection pressure during a one <br />hour period, the well shall be automatically shut-in. <br />(d) Fluid volume and flow rate: Magnetic flow meters and continuous <br />recording devices with a sampling frequency of at least every 15 seconds <br />EPA Draft Permit No. CO 10932-04664 Page 8 of 77 <br />
