<br />Gold Hill Mill 4 Mav 19. 1995 <br />fixtures and equipment to the State for reclamation, or if you would <br />rather post a bond sufficient to purchase geotextile. <br />12. It is the Division's view that the proposed facility monitoring <br />schedule should include more frequent verification of the water level <br />behind the Hazel-A bulkhead than the monthly monitoring you have proposed. <br />The water level behind the bulkhead should be observed and recorded daily <br />whenever the free water surface in the pond is high enough to over-top the <br />decant line stand-pipes. Also, monthly checks on the function of the <br />decant pipes is not sufficient. The decant risers should be observed <br />daily when the mill is in operation or when the free water pond is high <br />enough to over-top the stand-pipes, and the observations recorded. A test <br />that the decant line is clear, such as running hot water through the line, <br />should be performed monthly. As we have discussed, a monitoring log <br />recording all monitoring results and observations must be kept in the mill <br />office and available for review by the Division upon request. <br />13. Item 27 (d) of your May 2, 1995 submittal states that a pond liner <br />construction description letter would be attached. Such a letter was not <br />included in the attachments received by the Division, and will have to be <br />provided. <br />As we have discussed, water for the mill is not pumped out of the Wynona <br />mine shaft. Please provide a description of the actual system that is in <br />place. <br />14. As you know, a complete design and construction plan for the proposed <br />tailing dam raise, including quality assurance/quality control <br />specifications, will have to be provided for Division review in order to <br />approve the permit application. Also, as we have discussed, since a <br />downstream dam raise methodology is to be proposed, the Division will <br />require that the pond liner be extended up to the crest of the raised dam. <br />15. Rule 3.1.5(6) requires that all drill or auger holes that are part <br />of the operation be plugged. The Division will require bond to abandon <br />all site monitoring wells, including the Bureau of Mines wells that are <br />being used for monitoring, unless documentation can be provided that the <br />wells have been permitted as permanent features by the Division of Water <br />Resources. <br />16. Rule 3.1.5{6) requires that all adits and shafts be closed. Since <br />the Time mine adit and Hazel A adit are integral to the proposed COM, Inc. <br />operation, sufficient bond must be posted to assure that these openings <br />are adequately safeguarded. Locking metal gates installed in accordance <br />with the General Bid Specifications of the Colorado Inactive Mine Program <br />would be acceptable. Please incorporate a cost to close these two <br />openings into the reclamation cost estimate, and also provide the basic <br />dimensions of the openings for inclusion into the file. <br />17. Please revise your reclamation cost estimate to include the items <br />
