Before acceptance of <br />this insurance certificate, the Division would like four changes to be made to the document. <br />First, the certificate states that the insured is Costain Coal Co. Rule 2.03.911) states that the <br />certificate must certify that "the applicant has a public liability insurance policy in force for the <br />surface coal mining and reclamation operation...." Since Costain Coal Co. is the owner and <br />Sunland Mining Corporation is the applicant, please change the insured to the Sunland Mining <br />Corporation, along with its address. <br />Second, for the same Rule 2.03.9111, please add in the box entitled "Description of <br />Operators/LocationslVehicleslSpecial Items" that the insurance policy is for the Apex No. 2 Mine, <br />Colorado Permit No. C-81-011. <br />Third, pursuant to Rule 2.03.9131, please cross out, in the "Cancellation" box, the words <br />"endeavor to" and "but failure to mail such notice shall impose no obligation or liability of any <br />kind upon the company, its agents or representatives." <br />Fourth, please change the certificate holder from "Colorado Department of Mines and Minerals" <br />to "Colorado Division of Minerals and Geology." <br />After these changes have been made, please submit the new Certificate of Insurance. <br />If you have any questions, please call me. <br />Sincerely, <br />O~/ <br />seph J. dash v <br />Environmental Protection Specialist <br />JJDlbjw <br />cc: Greg Lewicki <br />\BJW\SUNLAND.JJD <br />
