The mine ie <br />reclaimed and the permit has been revoked. No facilities remain on site. No personnel are connected <br />with this mate site. <br />The focus of tins aspection was to follow trp on the two slides noted at Division of Minera]a and <br />Geology's May 11, 1998 inspection. Mr. Walker assessed the site in terms of engineering stability. <br />Mt. Walker's irrterofflce memo accompanies this report <br />The sfide West of Pit 4 did not appear to have enlarged since the May 11, 1998 inspection It appeared <br />that the run out of the sfide had reached Grassy Creek, but had not blocked the creek Grassy Creek <br />was flowing unimpeded at the time of this inspection. The dimensions of the mndflow are visually <br />estimated and paced at 40 R Wide by approx 120 R Lang with the tce of the slide spreading to -- 90 <br />R wide. W ater is flowing out of the slope of the slide at approx Less than 3 gallons per minute. <br />Investigation of Pond 4 at the base of Pit 4 fmde the riser undercut and piping along riser to have <br />occnved Piping ie not apparent on outslope of embankment Jnat below the pond embankment on the <br />west ode of the pond ontalope vegetation consists of very dense oak brush. Thorough investigation of <br />the slope below the pond is limited by the dense vegetation The head of the scarp below the pond was <br />accessed. Read scarp origatabes approx 70-90 R Below the pond embankment. GuIlying in slope <br />above the scarp and below the pond embankment was observed. Side head scarp is approx 5 R in <br />height A eedimerrt delta apparently from the run ant of the flexible pond riser sleeve has accumulated <br />at this location. A epmng from the hillalope also exhibits itself here. Vieaally it appears that natural <br />causes cantnbute to the founation of the slide, however, contnbution of concentrated water from Pand 4 <br />may add to the inslabtlrt~ y of the slope. <br />Hairpin tnm on the access road to Pit 4 coattittaes to cut <br />P~arg of the ditch along access road to Pit 2/3 has occt~ }gip phtcement in the locale mey <br />provide necessary reinforcement.
