Please check one below n remm this <br />form with your payment to DMG. If a CD is requested and the permit is too large for a CD, you will be called. <br />Please allow up to 5 business days from the receipt of this signed dacumrnt for your CD or DVD to be available. <br />~CD ~ 55.00 per disk _ DVD Q $20.00 per disk <br />Please check if you want a copy of the QnickView installation instructions. <br />Computer requirement to nee the DMG document Imaging CD (or DVD): <br />1. A program called QuickView will be copied to the CD and must be installed on your computer in order to use a DMG imaging CD the <br />first time. Futuro use of this or other imaging CD's does not require a new installation of the software. <br />2. The CD or DVD that you purchase mwt be in the D: drive. A DVD drive will read a t~ or DVD. <br />3. The CD will contain files with the .tif extension. Thera needs to be an image viewing application for .tif files on your computer that <br />will handle single and multi-page documema. On a Windows XP computer, the default viewing programs are called "Windows Picture <br />and Fax Viewers' or "Palm" and they do not work with QttickView. The hnaging Viewer that comes with pre-Windows XP computers <br />does work. DMG has decetmined that a program called "LfanV iew", which cau be downloaded from hfanView.com, does work with <br />QuickView. After downloading "IrtanView", it needs to be configured as yom default .rif viewer. DMG does not support or endorse this <br />software and the licensing and registration are the user's rbsponaibility. Them are many other programs drat work ae well, such u <br />"Imaging Preview" and "Imaging for Windows". <br />Iostructlops• <br />1. Insert the CD into your cemputer. <br />2. Allow time for the CD to install. <br />3. Eject the CD from the wmputer. <br />4. Reinsert the CD into the computer. <br />5. You are ready to view the Permit Files. <br />NOTE: A program called QuickV iew Autorun, wMch tuna the QttickViaw <br />setup program, has also been copied to this CD. if the Aucetun program <br />does NOT work, you will need to manually install the QuickView program. <br />Instructions for manually installing /ZnickView are available upon request <br />with the CD or can be viewed at www.minina.atace.co.us. <br />The Permit File document images can be viewed In the receptlon area of rho DMG ofliee or they can be purchased on a CD or <br />DVD. The CD/DVD purchase tort coven the tlme and labor of making the CD.
