-3- IA. Revisions Reouired 1. Legal, Financial, Compliance Information - The Division finds that the following information is inaccurate and needs to be updated to reflect changes that have occurred over the past 2 1/2 years. Specifically: a. A revised table of contents should be submitted for each volume of the application. The table of contents in Volume 1 should include all additional or revised exhibits and maps and should include a list of all supplemental material that has been submitted, including stipulation responses. b. Page l-3, Figure l.l.A should be revised to include the 5 year permit boundary on the right of entry map. c. Table 1.3.A, Status of Permits, should be updated to reflect the current status of all permits. d. The permit application form found on page 1-117 should be revised to reflect current names, addresses, contacts, acreages, etc. e. The current status of the right of entry statement No. 13 and No. 14 on page 1-10a should be indicated. f. The list of Directors and Officers found on page 1-167 should be brought up to date. 2. Hydrologic Balance - a. There have been changes and additions to the drainage and sediment control plan for the mine. The following items should be included in a revised drainage control plan that includes appropriate maps, designs, calculations, and narrative: that can be incorporated and consolidated into the appropriate section of the application. 1. Drainage control for the area NW of the silo including ditch 21. 2. The revised dewatering mechanism for Pond P4B No. 2. 3. The ditch and pipe behind the mine office that diverts water from above the laydown area. 4. The collection pipe at the spring below the powder magazine that leads to ditch 10. 5. The proposed storage yard and ditch east of the portal. 6. The drainage culvert and proposed berm configuration near the outslope at the breaker building.
