Legal, Financial, Compliance Information - The Division finds that the <br />following information is inaccurate and needs to be updated to reflect changes <br />that have occurred over the past 2 1/2 years. Specifically: <br />a. A revised table of contents should be submitted for each volume of <br />the application. The table of contents in Volume 1 should include all <br />additional or revised exhibits and maps and should include a list of all <br />supplemental material that has been submitted, including stipulation <br />responses. <br />b. Page l-3, Figure l.l.A should be revised to include the 5 year <br />permit boundary on the right of entry map. <br />c. Table 1.3.A, Status of Permits, should be updated to reflect the <br />current status of all permits. <br />d. The permit application form found on page 1-117 should be revised <br />to reflect current names, addresses, contacts, acreages, etc. <br />e. The current status of the right of entry statement No. 13 and No. <br />14 on page 1-10a should be indicated. <br />f. The list of Directors and Officers found on page 1-167 should be <br />brought up to date. <br />2. Hydrologic Balance - <br />a. There have been changes and additions to the drainage and sediment <br />control plan for the mine. The following items should be included in a <br />revised drainage control plan that includes appropriate maps, designs, <br />calculations, and narrative: that can be incorporated and consolidated <br />into the appropriate section of the application. <br />1. Drainage control for the area NW of the silo including ditch 21. <br />2. The revised dewatering mechanism for Pond P4B No. 2. <br />3. The ditch and pipe behind the mine office that diverts water <br />from above the laydown area. <br />4. The collection pipe at the spring below the powder magazine <br />that leads to ditch 10. <br />5. The proposed storage yard and ditch east of the portal. <br />6. The drainage culvert and proposed berm configuration near the <br />outslope at the breaker building. <br />
