<br />Item'4 - In Appendix A, the industries are listed by their SIC Code. (A two digit code, such as 22, means <br />that'all industries under that heading, such as 2221, 2249, etc., aze covered.) Use this table to <br />determine which of the Division's general permits is appropriate for your facility. <br />If your facility is not covered by general permit A (Light Industry), general permit B (Heary <br />Industry), or general permit M (Metal Mining), but by one of the other permits, you will need to <br />obtain the appropriate application from the Division. <br />For mines, the size/status category is necessary in order to determine the fee category for the facility. <br />Inactive mines aze mining operations that aze not being actively mined. Coal mines, both active and <br />inactive, which are not covered under the Division's general permit for surface coal mining (COG- <br />850000) may use this application to apply for coverage under the Metal Mining general permit. <br />Item 5 - Identify the receiving water. Receiving waters are any waters which aze contained in or flow in or <br />through the state of Colorado (except for water withdrawn for use until use and treatment have been <br />completed). This definition includes all water courses, even if they are usually dry. <br />If stormwater from the industrial site enters a storm sewer system or ditch, identify that system and <br />indicate the ultimate receiving water for the storm sewer or ditch. Note: a stormwater discharge <br />permit does not allow a discharge into a ditch or municipal storm sewer system without the approval <br />of the owner/operator of that system. <br />Item 6 - Describe the primary industrial activities at this facility (e.g., trucking firm with vehicle maintenance, <br />precious metals mine, computer equipment manufacturer, etc.). Indicate whether or not the facility <br />has a coal pile. If this application is for any of the following types of facilities, also provide the <br />additional information indicated: <br />Airport: state the estimated volume of deicers used, and the volume of fuel sold, on an annual basis. <br />Wastewater treatment plant: include the design flow, service population, and pretreatment program <br />status. <br />Steam electric power plant: indicate the primary and backup fuel sources. <br />Paving and roofing materials manufacturing: indicate whether or not the facility manufactures <br />asphalt emulsion. <br />Asphalt or concrete batch plant: Indicate whether or not the plant is portable. <br />Item 7 - Indicate the environmental permits the facility has or other environmental programs to which you aze <br />subject. <br />a) If you currently have a discharge permit from the Division for the discharge of process water, <br />include the number here. (CDPS -Colorado Discharge Permit System; same as NPDES - <br />National Pollutant Dischazge Elimination System) <br />b) If you aze under the RCRA program, list your EPA identification number, and indicate whether <br />or not you aze classified as a TSD (Treatment, Storage or Disposal) facility. <br />c) Self explanatory <br />d) Indicate whether your facility is subject to reporting requirements under SARA (Superfund <br />Amendments and Reauthorization Act) Title III, section 313, toxic chemical release reporting <br />(Toxic Release Inventory, Form "R"). (Note that Section 313 is not the same as Tier 2 <br />reporting, which is under Section 312.) <br />7/96/Ihm -ii- <br />
