<br />Mr. Duane Woodward <br />September 16, 1987 <br />Page 3 <br />5. Nature and Extent of Injury: The Claimant Dale Ackerman <br />received severe injuries. The primary injury was the loss of his <br />leg which was amputated while he was trapped inside the cab of a <br />haulage vehicle. He also suffered an injury to his right arm <br />which has required surgery. A claim for damages will include <br />recovery of his expenses, damages for physical and mental pain <br />and suffering, damages for past and future economic loss, damages <br />for past and future loss of enjoyment of life, damages for future <br />pain and mental suffering, and any other special damages to which <br />the claimant may be entitled. In addition to the damages <br />suffered by Dale Ackerman, his wife, Melody Lee Ackerman, has <br />suffered damages for loss of consortium. <br />6. Amount of Monetary Damages Being Claimed: The full nature <br />and extent of the Claimant's injuries and damages are not yet <br />known. The Claimant Dale Ackerman anticipates filing a claim for <br />not less than $5,000,000.00 in actual damages.
