4.1 APPENDIX A -AMENDED DECISION RECORD <br />FOR WOLF MIDGE CORPORATION <br />PILOT S(:ALE S(~IUM SOLUTION MINE <br />The decision is to amend the Decclion Record of May <br />2, 1986, for the subject project az regards stipulation 5 in <br />the Well Drilling, Completion, and Abandonment section <br />within Exhibit A. The fifth stipulation is revised to read <br />az follows: <br />The completion procedure will utilize 100 feet of <br />annular cement at the bottom of the drill hole with a <br />a~mpetent casing pack above the annular cement to <br />surface. This procedure is authorized under the following <br />conditioas: <br />1. This completion technique is audtorized for the initial <br />well pair only. <br />2. Prior to commencement of operations, WRC submits <br />to the District Manager, a monitoring program that <br />addresses the following objectives;: <br />a. That the presence of mud gel behind the casing is <br />detectable prior to, and after, sodirtm mining operations, <br />and <br />h. That an adequate hydraulic seal is maintained over <br />the operating life of the wells. <br />c:. The District Manager may require that additional <br />monitoring be performed where WRC's program <br />appears inadequate to demonstrate whether these <br />objectives will be met. <br />3. <br />4. <br />5. <br />The purpose of this WRC single well pair operation <br />is to demonstrate adequacy of the program. Accord- <br />ingly, the District Manger shall be advised of all <br />operations so that he may arrange for appropriate <br />monitoring of operations by the BLM. <br />The District Manager may require that operations cease, <br />or that remedial procedures be implemented a[ any <br />time that his monitoring of your operations determines <br />protection of the hydrologic or other resources is not <br />occurring. <br />The use of mud gel for completion purposes on <br />subsequent wells will only tx considered if the <br />monitoring results show that the material is an adequate <br />substitute for cement and does provide an adequate <br />seal to ensure protection of groundwater and other <br />resources. <br />This stipulation is being revised based upon the review <br />of additional information submitted by WRC which shows <br />that a sufficient probability of achieving the resource <br />protection required by the lease terms exists. <br />Stipulation Dumber 2 regarding [he plugging and <br />abandonment procedures is upheld. The use of the mud <br />gel in the completion technique will allow for the recovery <br />and reuse of most of the using. However, az required by <br />the District Manager, cement must be used in the plugging <br />and abandonment of these wells, <br />The remainder of the Decision Record of May 2, 1986, <br />is unchanged and continues to be the bureau's decision az <br />regards the plot project. The revision of stipulation 5 in <br />the Well Drilling, Completion and Abandonment section <br />of Exhibit A, az outlined above, will not result in significant <br />impacts to the environment, therefore, an environmental <br />impact statement is not necessary. <br />/s/ Cecil Roberts <br />State Director <br />September ]0, 1986 <br />Date <br />t aecauu of the change in the well completion stipulation, the plugging <br />sad abandonment stipulation requved wording changes sad now reads: <br />A "Nona of Intent to Abandon" will be submitted by the designated <br />operator/Issue to the authorized oltiar before abandonment of any <br />well developed within this projett 7Le notice will contain an "ss- <br />built" diagram of the well asd will describe any changes from the <br />approved abandonmrnt/plugging procedures. 1Le authorized officer <br />will review and approve or approve with modifications the notice <br />within IS calendar days of receipt. No sperial form for this notice <br />h required. AS a minimum, [he following plugs will be required: 1) <br />a steel bridge plug will be placed at the base of the produrtion casing; <br />2) 100 feet of amens will be plead above this steel bridge Plug; <br />3) a amem plug will be placed 20 tat above the Mahogany Zone <br />down through m 120 tees into the Mahogany Zone and must be <br />tagged to insure placement; and 4) 65 tat of amens will be placed <br />at the surt~ce. 7Le inurvaLS betwan the cement plugs wi0 be filled <br />with 9 pound/gallon or heavier mud. (Aber cement plugs or revised <br />Pluming Procedures may be required, baud on analysis of the cement <br />evaluation logs and the temperature survey log. An appropriate surtece <br />hole location marker will be installed at grade. <br />4.1-1 <br />
