Please check one below ad8 return this form with your payment to DMG. If a CD is requested and [he permit is too large for a CD, you will be called. Please allow up to 5 business days from the receipt of this signed document for your CD or DVD to be available. CD @ $5.00 per disk _DVD @ $20.00 per disk Please check if you want a copy of the QuickView installation instmctions. Computer requirements to use the DMG document imaging CD (or DVD): 1. A progam called QuickView will be copied to the CD and must be installed on your computer in order to use a DMG imaging CD the first time. Future use of this or other imaging CD's does not require a new installation of [he software. 2. The CD or DVD [ha[ you purchase must be in the D: drive. A DVD drive will read a CD or DVD. 3. The CD will contain files with the .tif extension. There needs to be an image viewing application for .tif files on your computer that will handle single and multi-page documents. On a Windows XP computer, the default viewing programs are called "Windows Picture and Fax V fewer" or "Paint" and they do not work with QuickView. The Imaging V fewer that comes with pre-Windows XP computers does work. DMG has determined that a program called "IrfanView", which can be downloaded from IrfanView.com, does work with QuickView. After downloading "IrfanV few", it needs to be configured as your default .tif viewer. DMG does not support or endorse this softwaze and the licensing and registration are the user's responsibility. There are many other programs that work as well, such as "Imaging Preview" and "Imaging for Windows". Instructions 1. Insert the CD into your computer. 2. Allow time for the CD to install. 3. Eject the CD from the computer. 4. Reinsert [he CD into the computer. 5. You are ready [o view the Permit Files. ~, NOTE: A program called QuickView Automq which mns the QuickView setup program, has also been copied to [his CD. If the Automn program does NOT work, you will need to manually install the QuickView program. Instmctions for manually installing QuickView are available upon request with the CD or can be viewed at www.mininestate.co.us. The Permit File document images can be viewed in the reception area of the DMG office or they can be purchased on a CD or DVD. The CD/DVD purchase cost covers the time and labor of making the CD. DMG cannot provide computer related support.
