<br /> <br />Gold Hill Mill 8 March 27, 1995 <br />(b) monitoring of the water level and available storage capacity in <br />the underground water storage reservoirs; <br />(c) observations of underground water storage facility bulkhead <br />stability; <br />(d) observations of the function of the tailing impoundment decant <br />pipes; <br />(e) observations of the tailing impoundment to verify adequate <br />freeboard and monitor water balance, assess impoundment stability <br />and security, and to verify liner integrity; <br />(f) observations of the tailing line from the mill to the <br />impoundment to assure that it is intact and not leaking; <br />(g) observations of mill building tanks and other containment <br />facilities; <br />(h) and, observations of the water line from Left Hand Creek to <br />assure that it is intact and not leaking, as well as observations of <br />all water handling facilities to assure that they are functioning <br />properly and to monitor for controlled water balance. <br />27. COM, Inc. must provide a complete description of the existing <br />embankment, to include: <br />(a) foundation characteristics, including topographic configuration <br />as well as foundation surface preparation; <br />(b) .dimensions of the existing dam, including crest elevation, <br />crest width and length, upstream slope angle, and downstream slope <br />angle. Cross-sections would be useful in this regard; <br />(c) provide a characterization of the exiting embankment materials, <br />including grain size distribution, density, and shear strength, also <br />describe the construction methods employed to build the dam if this <br />information is available, including any quality assurance <br />information; <br />(d) provide information available on the existing liner, including <br />any quality assurance/quality control information, subgrade <br />preparation, and also provide a time frame and methodology for <br />patching flaws in the existing liner; <br />(e) provide a complete description of the decant lines and <br />underground water storage facilities. <br />28. COM, Inc. must provide a description and pertinent analysis on how <br />the dam will be raised to provide additional storage capacity for both <br />
