Collins : <br /> OrrMay-21- 1996, we -received your proposal to modify yuui existing, <br /> approved mining plan of operations known as the Joker Mine project <br /> (ELM casefile number CO-016 -PO- 83 - 1) . We have also received and <br /> reviewed the related NEPA required Environmental Assessment (EA) <br /> document submitted by Knight Piesold of Elko, Nevada. After a <br /> thorough review of the plan amendment and the EA, Mining and Minerals <br /> II, Ltd. , having been recognized as the operator of record for this <br /> plan of operations, is hereby given authorization to proceed with the <br /> plan amendment modification as proposed. Specifically, this involves <br /> the drilling and installation of two ground water monitoring wells and <br /> the backfilling of rinsed, processed tailings material transported <br /> back to the mine site from the processing facility at Baggs, Wyoming. <br /> Please find enclosed, Attachment A, Special Stipulations, which apply <br /> to this authorization as approved. These stipulations will be <br /> monitored and enforced during your period of operations . Please <br /> notify this office of any and all changes or modifications to your <br /> operating plan and of the results of your pilot plant testing and <br /> ground water monitoring sampling. Again, be advised that Elkhead is <br /> liable for all past, present, and future reclamation liabilities <br /> associated with this approved mining plan and it's subsequent approved <br /> plan amendments .
