PLEASE TAKE NOTICE, that the Division of Mined Land Peclamation of the
State of Colorado Department of Natural Resources has determined, pursuant to
C.R.S. 34-33-123(7), that a pattern of violations exists as to the
requirements of the captioned permit, or as to the requirements of Article 33
of Title 34, Colorado Revised Statutes, at the captioned mine.
Furthermore, the Division has found that:
~_ Such violations are caused by the unwarranted failure of the Permittee
to comply with such requirements,
_ Such violations are willfully caused by the Permittee.

The Mined Land Reclamation Board will hold a public hearing on this
Order. You will be notified of the time and location of the hearing 30 days
prior to the date of the hearing.
