Long <br /> Divivon Director <br />Re: PikeviewQuarry,AmendmentApproval,PermitNo.M-1977-211,RevisionNo.AhI-2 <br />Dear Mr. Paradav: <br />On March 15, 2001 the Division of Minerals and Geology approved the Amendment applicationsubmttted to the <br />Division on September28, 2000, addressing the following: <br />The amendment reduces the overall slope of the Lay back area to 1. Sh: l v; adds 26 acres of Federal Land <br />to the permit area, and 14. lofthose acres are affected land <br />The terms of AmendmentNo. 2 with the following two (2) conditions are approved by the Division and are hereby <br />incorporated into Permit No, Iv1-1977-211. All other conditions and requirements of Permit No. lvt-1977-211 <br />remain in full force and effect. <br />Conditions of Permit Approval <br />- - --- Condition No. 1 <br />No portion of the quarry highwall will be constructed (mined) to slope angles steeper than the <br />granite eastjoint dip without prior approval by the Division. Castle Concrete will report to the <br />Division, as soon as the granite is exposed, the east joint dip and the bench highwall slope angle <br />for each cliff (bench highwall) at approximately the north and central ridee locations. Castle <br />Concrete will report the dip and slope angle at these two locations for each new bench highwall <br />until the final granite portion of the highwall configuration is completed. The report will be <br />prepazed and signed by a registered geotechnical engineer or other qualified zngineer, and <br />include an analysis of the stability of that cliff (bench highwall) as it relates to the overall <br />stability of the amendment area quarry highwall. If such an analysis reveals potential highwall <br />stability problems, Castle Concretz will propose a modification to the mining plan to eliminate <br />the instability problem. <br />Condition No. 1 <br />Castle Concrete Company will not disturb National Forest Land proposed to be affected under <br />Amendment No.3 until the US Forest Service provides in writing the source of Castle Concrete's lesal <br />right to enter the land to conduct mining operations and that document is reviewed and approved by the <br />Division. Furthermore, Castle Concrete Company will not affect its own land in a manner proposed <br />
