JUDE MINE, PERMIT M-1978-039 HR <br />March 2003 <br />,.- - <br />Received <br />MAID 2 7 20Q3 <br />Durango Field Office <br />Divisi0~ of Minerals & Geology <br />The St. Jude Mine is one of five permitted mines held by the Operator in the Big <br />Gypsum Valley of San Miguel County, Colorado. The St. Jude Mine permit includes the <br />mine portal, mine yard, support building, electric power service, topsoil stockpile, waste <br />dump, and related access road. There are no vent shafts or other outlying features included <br />under the St. Jude Mine 110 Permit. <br />Site Features. <br />The enclosed map, ST. JUDE MINE PERMIT M-1978-039 HR, 1" = 100', shows the <br />features at the St. Jude mine site. All features in the Permit area have been re-surveyed, and a <br />number of differences exist between this current map and the existing map on file with the <br />Division. The current map shows features which have been added since the old map was <br />submitted; added features include waste dumps, diversion structures, runoff catchment berms, <br />etc. <br />Roads. <br />Primary access into and through the site is via a dedicated access road which extends <br />from the west side of the Carnation mine site. On the 1" = 100' site map, the access road is <br />shown as extending to the east to the point at which it meets the Carnation Mine permit <br />boundary; this configuration matches the existing 1978 site map. <br />The access road is in good condition. In November 2002 the Operator graded the <br />access road. <br />Buildines and Structures. <br />Mine facilities include the St. Jude portal, one metal shop/dry building, an above <br />ground 500 gallon fuel storage tank, two buried water storage tanks, loading docks, and a <br />power line and a transformer station. <br />The existing shop/dry building is in generally fair condition. The building is 50' x <br />40'. <br />The portal was used during the period of November 1997 through June 1999. The St. <br />Jude Mine has remained on "standby" since that time. In November 2002, the Operator piled <br />dirt in front of the portal to prevent access into the portal and mine. <br />In November 2002, the Operator cleaned up all trash and debris that had accumulated <br />around the site. <br />Site Power. <br />One power line and one substation exist within the Permit, and one line is adjacent to <br />the east side of the Permit. This second line provides power to ventilation and air compressor <br />installations on the ridge above the mine site. The transformer platform in the mine yard is <br />stable and secure. The transformers are not tagged with light blue tags used by a former <br />operator. Therefore, these transformers likely belong to San Migue] Power Co. Ownership of <br />all electrical installations in the St. Jude area needs to be verified with San Miguel Power. A <br />request has been submitted to San Miguel Power to assist the Operator in correctly identifying <br />
