At all limes, including periods of start-up, shutdown, and malfunction, the facility and control equipment shall, to the <br />extent practicable, be maintained and operated in a manner consistent with good av pollution prevention and control <br />practices for minimi>ing emissions. Determination of whether or not acceptable operating and maintenance procedures <br />aze being used will be based on information available to the Division, which may include, but is not limited to, monitoring <br />results, opacity observations, review of operating and maintenance procedures, and inspection of the source. <br />6. Construction /Modification of the sources listed under AIRS POINT Ids: 158, 159, and 163, must commence within IS <br />months of initial approval penni[ issuance or within 18 months of the start-up date stated in the application. If <br />commencement does not occur within the stated time the permit will expire on April 30, 1998. <br />(See General Condition No. 6., Item l on the reverse side of the fast page of this permit.) <br />7. The activities at this facility shall be limited to maximum production / wnsumption /processing /operation / handling rates <br />as listed on Attachment A. Shott term limits (per day) shall be annual limits divided by 143. Records of the actual rates, <br />and emissions of criteria and noncriteria reportable /hazardous av pollutants shall be maintained by the applicant and <br />made available to the Division for inspection upon request. Such records shall, at the minimum, contain the details given <br />in Attachment C. <br />8. Emissions of a'u pollutants shall not exceed the following Iunitations (as calculated in the Division's preliminary analysis): <br />Particulate Matter :..................... 159.0 tons per year and 2,224 pounds per day <br />Particulate Matter < 10 µm (PM~~:......... 85.0 tons per year and I, l89 pounds per day <br />Note I: In addition, the above activities also emit the following fugitive drat, and these have been entered into the <br />Division's emission inventory system: <br />Particulate Matter -Fugitive: 319.0 tans per year and 4,448 pounds per day <br />Particulate Matter < 10 µm (PM~o): 83.0 tons per year and 1,161 pounds per day <br />Note 2: The above emissions are based on emission factors and control efficiencies given in Attachment D. <br />Note 3: The permittce shall identify emissions of noncriteria repotable J hazardous air pollutants. ]f emissions of such <br />pollutants exceed the notification thresholds, Av Pollutant Emission Noticels shall be submitted within one (1) <br />month of the issuance of this pemtit. <br />9. Compliance with annual limits shall be determined on a rolling twelve (12) month total. By the fifteenth day of each <br />month a new twelve month total shall be calculated using the previous twelve calendar months' data. The permit holder <br />shall calculate monthly emissions and keep a compliance record on site for Division review. <br />10. Prior to final approval being issued, the applicant shall submit to the Division for approval a revised facility-wide <br />compliance plan and record keeping format that will outline how the applicant will maintain on-going compliance with <br />the requirements of this permit. This includes the control efficiencies specified in Attachment A. <br />I I.
