Please chxk one below mad retain this <br />form with your payment to DMG. if a CD is requested attd the permit ix too large for a CD, you wt71 be called. <br />Please allow up to 5 business days 5rom the receipt of this signed document for your CD or DVD to be available. <br />~ CD (a3 $5.00 per disk ` DVD Q $20.00 per disk <br />,Please check if yon warn a copy of the QuidcView installation inslrtrctions. <br />Computerregtrirements to use the DMG document imaging CD (or DVD): <br />l . A progwm called tluickView will be copied to the CD and must be installed on your computer in order to use a DMG imaging CD the <br />first time. Future use of this or outer imaging CD's does twt require a new instagation of the soiware. <br />2. The CD or DVD that you puxcbase >~ be in the D: drive. A DVD drive will read a CD ar DVD. <br />3. The CD will contain files with the .tif extension. Thane needs to 6e as image viewing appHcatioo for .tif files on your computer that <br />wilt handle single and rnutti-page documents, On a Windows XP computer, the default viewing programs ate called "Windows Picture <br />and Fax Vicwcr" or "Paine' sad they ~ with QuickView. The Imaging Viewer drat comes with praWindows XP computers <br />does work. DMG has determined that a program called `TrfanView", which can be downloaded from IrfnaView.com, does work with <br />QuickView. After downloading "IrfanView", it needs to be configured as your default .tif vicwa. DMG does not support or rndorsc this <br />software and the liceaging and registration arc the user's responsrbility. There are many other pmgtatm5 Otatworir as well, stclt as <br />"Imaging Preview" end "Imaging for Windows". <br />Instructiaas: <br />1. Insert flte CD 'into your computer. <br />2. Allow time for the CD to install, <br />3. Eject the CA from the caurputer. <br />4. Reinsert the CD into the computer. <br />5. You are ready to view the Permit Files. <br />NOTE: A program called QuiekView Antonin, which urns the QrticlcView <br />set»p program, has also been copied to this CD. If fire Atttorun program <br />does NOT work, you will need to roannally install the QuiekView program. <br />Inshactions for manually inafnl_ lino QttickView are available upon request <br />with the CD or can be viewed at www,mirtin¢.state.co.ns. <br />The Permit File dowmeatimages can be viewed is the t~eeepHon area of the DMG oH'ice or they can be purchased on a CD or <br />DVD. Tpe•CDIDVD purchase cost covers the time and labor of making the CD.
