Reporting Requirements Reporting -Active Mining Operations: The pennittee wi116e required to submit an Annual Report, covering January 1 through December 31 of each year, on their compliance with the SWMP. The Annual Report will be due to the Division on or before February IS of the following year (see Part I.E.1 of the permit). 2. Reporting -Inactive Mining Operations: The pennittee will be required to submit a Compliance Report. The report will be due to the Division on or before February 15, 2004, and will cover the period of time from pennit certification to December 31, 2003 (see Part I.E.1 of the permit). Reporting - Remediation Projects: The pennittee will be required to submit an Annual Report, covering January 1 through December 31 of each year, on the compliance with the SWMP. The Annual Report will be due to the Division on or before February IS of the following year (see Part I.E.1 of the pennit). Ail repots required for submittal shall be signed and certified for accuracy by the pennittee. (See Pan I.E.2 of the permit.) E. Monitorintt Sampling and testing of stonnwater for specific parameters is not required on a routine basis under this permit. However, the Division reserves the right to require sampling and testing, on a case-by-case basis, in the event that there is reason to suspect that compliance with the SWMP is a problem, or ro measure the effectiveness of the BMPs In removing pollutants in the discharge. F. Retention of Records The pennittee for active mining operations shall retain a copy of the SWMP required by this pennit at the mining site for the duration of this permit. The pennittee for inactive mining operations or mine-waste remediation sites shall retain a copy of the SWMP required by this permit, for the duration of this permit. For all types of operations, the permit[ee shall retain copies of all other reports required 6y this permit and records of all data used to complete the application to be covered by this pennit, for a period of at least three years. G. Annual Fee All permittees are required to pay the annual permit administration fee, as described in the Colorado !3'ater Quality Control Act. (See Pan ILB.10 of the permit.) H. Termination of pennit Certiftcation When a site has been reclaimed in accordance with the Colorado Division of Minerals and Geology (CDMC} requirements and the site has been released from State reclamation requirements, the operator of the facility may request termination. The requirements for termination are described in Par I.F of the permit. In the case where the site has no CDMG permit, certification under this permit may be terminated once raw material, intermediate products, byproducts, finished products or waste products have been removed or isolated from stonnwater and the site has been stabilized (with little evidence of soil erosion or other runoff problem) and revegetated. Speciftc criteria are listed in Part I.F. of the permit. Continued coverage may be required after reclamation or remediation is complete if the Division has shown or has reason to suspect that the stonnwater discharge contributes to a violation of a water quality standard.
