Long,
Division Director
The Division has received a copy of the renewed personal injury and property damage insurance
certificate for the Meeker Area Mines. As I discussed with Mr. Greg Lewicki, the insurance
certificate must be amended to comply with Rules 2.03.9(1) and 2.03.9(3) of the Rules and
Regulations of the Colorado Mined Land Reclamation Board. The changes required are as
follows:
1. The insurance certificate must list the Meeker Area Mines in the "Description of
Operations" section.
2. The cancellation clause currently states that the issuing company'rvill endeavor
to mail..:' notification of cancellation to the Division. The certificate should be
amended so that the term "endeavor" is deleted. The portion of the clause which
states that failure to notify the Division of cancellation or substantive policy
changes imposes no obligation or liability upon the company should be deleted.
Since the current certificate does not comply with Rule 2.03.9, a revised certificate should be
submitted to the Division no later that June 30, 1992. If you have questions or are in need of
additional information, please contact me.
Sincerely,
~ L7'1'ls~t~,~c-~
Susan L Morrison
Reclamation Specialist
c: Greg Lewicki, Greg Lewicki and Associates
