Rule 2.03.80) <br /> 1. In Tab 1, on page 1, it is stated that the loadout facility permit area consists of 134 acres, <br /> with 54 acres being disturbed. However, the application form for a permit states that there <br /> are 176.3 acres within the permit area, with 97.1 acres being disturbed. Please reconcile this <br /> difference and revise the appropriate pages. <br /> Rule 2.04.7(4)(b) <br /> 2. On pages 1, 2 and 5 of Tab 13, it is stated that surface water monitoring sites and NPDES <br /> outfalls are shown on Exhibits 12-1, 12-2 and Figure 13-1, located in Tabs 12 and 13. <br /> However, site HGSD3 is not shown on Exhibit 12-1, Facilities Map. In addition, <br /> Exhibit 12-2, Surface Water Control Plan Map, does not show site HGSD3, nor does the <br /> legend contain a surface water location symbol. Please reconcile the text with Exhibits 12-1 <br /> and 12-2. <br /> Rule 3.02.2(4) <br /> 3. Please update the reclamation cost estimate to reflect an adjustment for 1995 costs. Cost in <br /> need of adjustment include equipment, labor, structural demolition and revegetation. <br /> Enclosed, please find a copy of 1995 cost data worksheets that should be used in the <br /> estimate. Equipment sheets supplied to Hayden Gulch reflect the equipment selected in the <br /> 1992 reclamation bond estimate. These reference sheets are being supplied to Hayden Gulch <br /> for reclamation estimating purposes only. <br /> 4. The Division recently incurred cost for pond cleaning that became necessary at a site for <br /> which the performance bond had been revoked. As a result of this, it has come to our <br /> attention that reclamation costs should include costs for dredging of ponds over the ten-year <br /> liability period. Please review site-specific data at the Hayden Gulch Loadout to determine <br /> which ponds, if any, could reasonably be expected to need cleaning over a ten-year period, <br /> and how often they might need to be cleaned. Please revise the cost estimate accordingly. <br /> 5. The Division noted one revision to the permit that did not include a reclamation cost <br /> estimate. TR-01 added alluvial well HDGAL3 to the permit area. Please include a cost in <br /> the estimate to seal and plug alluvial well ADGAL3. <br /> 6. The previous bond estimate did not include a cost to remove diversion ditches at the <br /> HG Loadout. Please include a cost to remove all ditches from the permit area. <br /> Rule 4.03 <br /> 7. In Tab 12, on page 3, it is stated that there are 2400 feet of haul road, named HGLF-HRI, <br /> within the permit area. This length of haul road, as described, extends from County Road 53 <br /> to the loadout facility entrance gate. Per Rules 4.03.1(1)(d) and 4.05.2(4), please submit a <br /> road certification for that haul road. If a road certification is not possible due to the lack of <br /> -3- <br />
