During October 10-12, 2006 Division inspection, John Tinucci of Agapito Associates, who <br />attended the inspection, stated to Division personnel that he had been conducting geologic <br />monitoring at the Trapper Mine. Please discuss the type of monitoring that was Mr. Tinucci was <br />conducting, the reason(s) for this monitoring, the duration of the monitoring, the locations of the <br />monitoring stations, and the data obtained from this monitoring. Please locate stations used for <br />this monitoring on the "Geotechnical Monitoring Dewatering Wells" map. <br />6. One monitoring point on the "Geotechnical Monitoring Dewatering Wells" map is labeled <br />"Agapito" (label on map is illegible). Please discuss what was being monitored at this location. <br />Please provide data logs for inclinometers installed in the East Panel Area, and current data logs to <br />demonstrate findings regazding the slip surface or surfaces. Please provide a description of the <br />purpose of these inclinometers. Please discuss if any such inclinometers other than those <br />currently indicated have been removed in the past, and the reasons for their removal. <br />8. Three "seismic holes", numbered 71693, 71978, and 72752, aze identified on the "Geotechnical <br />Monitoring Dewatering Wells" map. Please provide a description of the purpose of these 3 holes. <br />Please confirm whether any other such "seismic holes" exist (or existed) in the East Panel Area. <br />9. Six continuous core holes are depicted on the "Geotechnical Monitoring Dewatering Wells" map. <br />Please provide any core data, driller's log data, and geologic logging data from these holes. <br />Please also confirm whether any other such "continuous core holes" exist (or existed) in the East <br />Panel Area. <br />10. Please provide one map that superimposes the azeal extent of the landslide on the areas in the East <br />Panel Area previously approved for disturbance under the approved CDRMS permit and under <br />any CDRMS exploration notices of intent. TMI's map scale of 1-inch to 400 feet would be <br />preferable, since many of the Trapper Mine permit maps are at this scale. Please place all <br />geotechnical monitoring site locations on this map, as well as on the "Geotechnical monitoring <br />Dewatering Wells" map. Please also locate the Tri-State powerline and the Trapper Mine pettnit <br />boundary on this map. <br />11. Please identify the azeal extent of the slide on a copy of the L/ML roof structure contour map. <br />12. As a reminder, we had asked in our letter of October 20, 2006 for receipt of any photographs <br />obtained, and any maps generated as a result of, an aerial survey of the mine site that was to have <br />been conducted by TMI on October 13, 2006. Please provide these materials as soon as possible. <br />Thank you for your assistance with this investigation.
