Box 41 <br />Rockvale, Colorado 81244 <br />RE: Notice of Award, Hawk's Nest East Revegetation Project, <br />No. RN-GOAL-MINES-6 <br />Dear Mr. Kessler: <br />This letter is to inform you that your company is the apparent low bidder on <br />Project No. RN-GOAL-MINES-6, the Hawk's Nest East Revegetation project. <br />To complete the process of bid award, you must send in the following <br />information within ten (10) days of receipt of this notice: <br />i. A certificate of insurance in the amounts given in Item 5 of the <br />General Conditions section of the bidding documents. The <br />certificate of insurance must show the State of Colorado as an <br />additional insured; and <br />2. A performance bond and a labor and materials payment bond in the <br />amounts of 100 percent and 100 percent respectively of the bid <br />amount (512,200.00); and <br />3. Completed bond forms (blank copy attached) for the above-referenced <br />bond. Per your request, I have enclosed a copy of the second bid <br />(only 2 bids were received) on this project for your bonding <br />company. <br />When the above documents have been submitted and found to be in order, a <br />purchase order will be issued and a notice to proceed will be given.
