The first Quarterly Report shall cover the period from the effective date of <br />the Suspension Notification through the end of that quarter. Subsequent <br />Quarterly Reports shall cover the periods of: January 1 through Mazch 31; <br />April 1 through June 30; July 1 through September 30; and, October 1 <br />through December 31. Each Quarterly Report shall be submitted to the <br />EPA Denver Office within thirty (30) days after the end of the quarter to <br />the address below. Temporary Plugging <br />The borehole and casing of wells placed into temporary suspension status must be <br />securely plugged to prevent any movement of the fluid into or between USDWs. <br />a. Temporary Plu~na Plan. All wells to be placed in temporary <br />suspension status are deemed to be candidates for future operation. Any <br />wells that aze not deemed to be candidates for future operations shall be <br />plugged and abandoned using agency approved methods in accordance <br />with Part II Section E of this permit. The permittee must temporarily plug <br />suspended wells according fo the following plan. <br />+ Immediately prior to temporary plugging, the tubing system must <br />be removed from the intermediate casing. <br />+ A cast-iron bridge plug (CIBP) must be set inside each string of <br />casing at approximately the elevation of the bottom of the short . <br />string casing. If it is not possible to run a CIBP inside each string <br />of casing due to irregularities, acement basket shall be run inside <br />the casing to establish a cement plug. The bridge plug or cement <br />basket will be set at least 25 feet below the Dissolution Surface. If <br />the bridge plug or cement basket cannot be set at least as deep as <br />25 feet below the Dissolution Surface, the permittee shall file an <br />approvable well repair plan with the Director within 30 days or the <br />well shall be plugged and abandoned per anEPA-approved <br />plugging and abandonment plan within 60 days. <br />+ The well bore will be filled with a conditioned 10 Ib. bentonite gel <br />or equivalent.
