To Complete the process of bid award, you must <br />send in the following information within ten (10) days of receipt <br />of this notice: <br />1. A certificate of insurance in the amounts given in Number <br />5 of the General Conditions. The certificate of <br />insurance must show the State of Colorado as an <br />additional insured. <br />2. A project schedule, as specified in Number 7 of the <br />General Conditions. <br />3. A performance bond and a labor and materials payment bond <br />in the amounts of 100 percent and 100 percent <br />respectively of the bid amount ($124,150). The bonds <br />must be submitted using the State forms. Copies are <br />enclosed. <br />When the above documents have been submitted and found to be in <br />order, a purchase order will be issued and a notice to proceed will <br />be given.
