Table 3-1, page T3-1-2, lists a Flagstaff, Arizona, address for Powderhotn Properties Company, <br />which is now outdated. Please update the table as appropriate. <br />2. The Notice of Violation Lisl provided in Table 3-2, needs to be updated to report current status of <br />all listed enforcement actions, and to include enforcement actions not currently listed in the table, <br />pursuant to Rules 2.03.5(3) and (4). Please update the fist as appropriate. <br />2.03.8 Permit Tenn Information <br />3. Please update the permit term information narrative on page 3~6 to reflect current status. <br />2.05.2 Operation Plan -Estimated Area (or Life of Operation <br />4. Exhibits 51 and 53 depict CRDA-2 final construction desi®r with six bench levels, a final <br />maximum elevation of approximately 5025 to 5030 feet, with the upper bench final surface graded <br />to drain to the southwest (toward the outslope) at 2%. Exhibits 9C arrd 18 depict the final <br />configuration for CRDA-1, and show a crowned design, with six bench levels and a final elevation <br />of approximately 5025 feet. Final design wnfiguration has m[ been achieved on either pile. Due <br />to the fact that a minimal amount of additional material would be added to either pile (from <br />cleanup of coal 5nes at various facility areas), it would appear drat final reclamation designs as <br />depicted on the exhibits, would not be achievable. As a result, the foal desigp exhibits will need <br />w be appropriately amended. <br />In addition, the Division is cencemed with the potential for failure of the CRDA-2 upper diversion <br />ditch due to mass wasting from the steep slopes above the ditch, or due to lack of maintenance <br />following final bond release. With the currently approved CRDA-2 pile configruation, significant <br />erosion of the refuse pile would likely occur if the ditch wen: to fail. It would appear that long <br />term stability of the pile could be enlanced by provision of a modified final topography for the lop <br />of the pile, to ensure that debris laden flood waters would not flow across the pile and discharge <br />over the outslope. Please evaluate this concern, and provide an amended final design, which will <br />address the concerns regarding long term maintenance and geomorphic stability. <br />2.05.4(2)(al Reclamation Plan Detailed Timetable <br />7. The reclamation plan timetable is referenced on page 14-1, Section (2)(a), and shown in detail in <br />Figme 14-1. Any anticipated modifications to the timetable as presented in the permit will need to <br />be included in the revision submittal, with supporting explanation <br />
