This is a report of findings and a request for information as a result of an internal review of the
Canadian, C-81-026 Applicant Violator System (AVS) databank and internal files. In this memo,
requests for documentation means copies of letters of appointment, letters of resignation, minutes
of board meetings, contracts, etc. However, certificates from corporate secretaries that cite specific
sources have been accepted.
Enclosure 1 is a schematic of organizations that "owns or controls" [Rule 2.03.4(3)] the applicant
according to information in the AVS databank.
Enclosure 2 is a summary of documentation in the Colorado Division of Minerals and Geology AV S
files. Note that no documentation for 1997 or 1998 is on file.
Enclosure 3 is the AVS databank information regarding Permit CO-81-026. Please note that the
begin dates for the SLURCO Corp as operator and Lawrence I Corte as agent are missing. Please
provide me with documentation for the begin dates for entry into the AVS databank.
Enclosure 4 is the AV S information regazding the SLURCO Corp. A number of entries are suspect.
Documentation in internal files only goes back to May 10, 1993, except for a letter of resignation
(Robert C. McHugh as Secretary) dated March 31, 1992. Recommend that the applicant be asked
to verify all begin dates and provide documentation for any corrections. The applicant should also
validate end dates. The end dates (terminations, resignations, etc.) after 10 May 1993 were, except
for those of 04/I 1/96, taken as the date of new appointments. Such is not necessarily so (there is no
documentation that specifically documents end dates except for the sole letter of resignation). No
documentation could be found in internal files to validate the 04/11/96 end dates as officers of the
SLURCO corporation. These entries had been entered into the AVS system, apparently from
computer screen prints of the retirement of Charles W. Battey as an employee of KN Energy, INC.
