(3)(c) Approval of PRO1 was apparently <br /> pp y given without concern for access <br /> pads to CRDA #2 or #3. Such roads are not indicated on Exhibit(51 or <br /> L2. None are identified nor are specifications provided in any table <br /> or with any narrative in the permit application package. A revision to - <br /> add these roads to the permit application must be made. In addition, l <br /> there is presently no culvert under the access road to CRDA #2 where it <br /> crosses the approved Perim to er containment ditch. Addition of such a � <br /> culvert and any others necessary to proper drainage of these roads <br /> should be made part of the revision and supported with calculations <br /> demonstrating proper capacity. Summary tables where mention of these <br /> roads and drainage structures is normally made in the permit <br /> application package should also be revised. <br /> r, <br /> There is no Exhibit A available to the Division as referenced on <br /> p. 149. Such shoulde provided and should be current. <br /> (4)(a) Table 2.05.3I p. 168 does not contain a listing of the <br /> diversions or containment ditches associated with CRDA #2, of the <br /> diversion of Coal Canyon drainage or of the diversions/catchment <br /> ditches associated with Ponds 4 & 5 at the Cameo Mine. The Table <br /> should be updated to include these structures. OVe <br /> Mention in the narrative on pp 170-171 should include the diversions <br /> and ditches for CRDA #2. <br /> Narrative on p. 173 needs to be revised and supplemented. All sediment p- <br /> ponds etc, are not covered in Appendix X, only Ponds 1 through 8 are. <br /> (4) (b) Sediment Ponds 1 & 2 are indicated on p. 177 to involve two <br /> natural depressions. As a result, these do not have the configuration <br /> dimensions of the designs apparently approved for these ponds in <br /> RA endix X. Drawings , supporting calculations and engineering <br /> certt i _ations need to be furnished to establish that these ponds do <br /> have adequate capacity and otherwise conform to the requirements of <br /> Rule 4.05 .6(3) . <br /> (4)(d ) The narrative on p. 171 should be revised to state which types <br /> of ditches are designed for 10-year, 24-hour events. Ditches <br /> associated with the refuse area are required to be able to pass � <br /> 100-year, 24-hour events. <br /> There are two culverts in the eastern containment ditch at CRDA #2. If <br /> these have a purpose and are intended to remain in the ditch, their <br /> capacity to pass the required flow in the ditch must be demonstrated <br /> and this information as well as evidence of the existence of the <br /> culverts must be included in appropriate parts of the application <br /> package. <br /> Additional sediment control items: The hay bale "berm" around the <br /> storage area at the Cameo Mine s ouId be added to Exhibit 8 B to 1 <br /> document the means of runoff control for this area which otherwise , <br /> would appear to lie outside the surface water and control system. <br /> -8- <br />
