LABORATORY DOCUMENTATION (Appendix D) <br />Chain of Custody forms which accompany samples for laboratory analyses are required to <br />contain the following information: site name and address, sample number, date and time <br />sampled, matrix description (e.g. soil, water), container size, type and decontamination <br />status (e.g. 40 ml laboratory decontaminated amber glass VOA vial), preservation method <br />(e.g. HCI S%, 4E C), date and method of transport to laboratory (e.g. hand delivered, Fed <br />Ex etc.), analyte and methodology of analyses requested (e.g. Total Extractable Petroleum <br />Hydrocarbons (TEPH) by EPA Method 8015 Modified), and the name and signature of the <br />person collecting the samples. Also attach all laboratory reports, and laboratory quality <br />assurance/quality control reports. <br />4. BOREHOLE LITHOLOGIC LOGS AND WELL COMPLETION (Appendix E) <br />Boring logs must include the site name and address, boring number, date completed, <br />surface elevation, depth, borehole diameter, initial and static water levels (if available), <br />drilling method, sampling method, lithologic graphic to scale, an indication of the intervals <br />from which samples were obtained, results of VOC screening, and a geologic description <br />and/or Unified Soil Classification System (USCS) class of each rock or soil type <br />encountered including any staining or petroleum odors. <br />The following additional information should be included with the lithologic logs if a well <br />has been completed. Include the total depth of the well, the casing elevation, the diameter <br />of the well casing, the well casing material (e.g. PVC, stainless steel), the length and depth <br />of the screened interval, the size and type of perforations of the screen (e.g. 0.010" milled <br />slots), the type of filter pack material placed in the well (e.g. 10-20 mesh washed silica <br />sand), the type of annular seal (e.g.1/2" bentonite pellets), the type of grout used to fill the <br />remaining annular space (e.g. Portland cement 5%bentonite), the type of well casing cap, <br />the method of securing the well casing cap, and the type of protective surface casing. Also <br />included should be a well completion graphic to scale which indicates the depth to the top <br />of the filter pack, the depth to the top of the annular seal, and the location of the screened <br />interval. <br />SCR - 02/01/99 <br />
