The name, address, and phone number of a contact person who will be <br />responsible for the operation and accounting of this plan must be provided to the <br />Division Engineer and Water Couunissioner. <br />8. This substitute water supply plan may be revoked or modified at any time should <br />it be determined that injury to other vested water rights has or will occur as a <br />result of this plan. <br />9. An application for a change in water rights and plan for augmentation must be <br />filed with the Division 1 Water Court within two (2) years of the expiration of <br />this year's plan (i,e., October 31, 1997) to include, but not limited to, long-term <br />evaporation losses. <br />This plan shall become effective through October 3l, 1995 upon written receipt of a <br />response from you within two weeks accepting the terms of this approval letter. If this plan is <br />not made absolute by a court action by the expiration date (i.e., October 31, 1995) an annual <br />renewal of the plan is required. Please contact Dick Wolfe in Denver at 866-3581, or Dave <br />Nettles in Greeley at 352-8712 if you have any yuestiorrs concerning this approval.
