Please be advised that the only way the Permittee Name can be <br />changed is by submitting a Succession of Operator Application, the application fee of $100.00, a new fmancial <br />warranty, in the amount of $11,500.00, in the name of the new per-mittee and a new performance warranty. <br />Until the Division receives and accepu all of the above Avalanche Creek Marble 8c Alabaster LLC will remain <br />the Permitted Operator for Permit No. M-1999-042.
