BANTA, Dlr~etor June 22, 1989 Mr. Murari Shrestha Senior Engineer Western Fuels - Utah, Inc. 405 Urban Street, Suite 305 Lakewood, Colorado 80228 RE: Required Submittals for Previous Field Minor Revision Approvals, Permit No. C-81-018 Dear Mr. Shrestha: Based on a review of the Division's files for field minor revisions MR 41, 42, 44, 45, and 46, it appears that applicable map and permit application text changes required by the approvals have not been received by the Division. The required submittals are as follows: hIR 41 - Install haul road crossing near guard shack, Submit revised map (No. 27) showing location of crossing. MR 42 - Install light duty road segments for access to powerline transformers. Submit revised maps (No. 27, 77, and 109) and revised text and bond talcs as necessary. MR 44 - Install temp road base mat. stockpile area between refuse sub-areas 1 and 2-3. Submit revised map No. 77 showing stockpile boundaries and sediment control structures; submit revised text as necessary. MR 45 - Install roadbase mat. stockpile bin at East Intake facility. Submit revised Map No. 85 showing bin location and sediment control alterations; submit revised text as necessary. MR 46 - Move refuse sub-area No. 2-3 equipment storage shed. Submit revised map No. 77 showing new shed location and any alterations to sediment control system. Please send two copies of the appropriate maps and revised permit application text to the Division as soon as possible.
