Dept. cf idatural Resoerces <br />I would like to express my sincere apology for the <br />incident that occurred at the Energy Mine on August 13, <br />1980. <br />An explanation of the circumstances surrounding the <br />incident should not be interpreted as an attempt on our part <br />to make excuses because there, of course, can be no excuse <br />for the failure to properly block a shot. <br />The results of our internal audit on this incident are <br />as follows: <br />1. Amine foreman checked the access road but, <br />since there was a berm and the road had not been used for <br />quite some time, did not assign a person to block it. <br />2. The person in charge failed to sound the siren <br />3 minutes prior to the blast. <br />3. The Company person accompanying you did not <br />have in his possession a working radio so as to be aware of <br />blasting activities while in the field. <br />The measures we will take to insure that this type of <br />incident does not reoccur are as follows: <br />1. All access roads into a blasting area will be <br />blocked by a guard. Furthermore, blasting areas will be <br />posted with signs and non-road access restricted with berms, <br />gates and other measures. <br />2. The person in charge will be responsible for <br />clearing the blasting area, contacting guards at access <br />
