<br />Document Imaging CD/DVD RECEIVED <br />REQUEST FORM & INSTRUCTIONS <br />Request for the DMG document images on [he following DMG permit file(s): <br />1) 1 58 2> (L1 (999D23 s> 11~19SIDIN <br />NOV 0 92005 <br /> <br />The DMG document images caa be copied to either a CD or a DVD. Please check one below and remm this <br />form with your payment to DMG. If a CD is requested and the permit is too large for a CD, you will be called. <br />Plea/se allow up to 5 business days from the receipt of this signed document for your CD or DVD to be available. <br />y_ CD @ $5.00 per disk DVD @ $20.00 per disk <br />Please check ifyou want a copy of the QuickView installation instructions. <br />Computer rtgnh-emenb to ast the DMG dornmeat imaging CD (or DVD): <br />1. A program called QuickView will be copied ro the CD and must fie instilled on yom computer in order to use a DMG imaging CD the <br />first time. Futmc use of this or other imaging CD's does not require a new installation of the software. <br />2. The CD or DVD that you purchase must be in the D: drive. A DVD drive will read a CD or DVD. <br />3. The CD will contain files with the .tif extension. There needs to be an image viewing applicakion for .tif files on yom computer that <br />will handle single and multi-page dcenmeats. fht a Windows XP computer, the default viewing programs are called "Windows Picture <br />and Fax Viewer" or "Paint" and they do not wodc with QuickView. The Imaging Viewer that comes with pre-Windows XP computers <br />does work. DMG has determined that a program cared "itfanView", which can be dotwloaded from LianView.com, does work with <br />QuickView. After downloading °IdanYiew", it needs to be configured as yom default .tif viewer DMG does not support ar endorse this <br />rwftware and the licensing and registration am the user's responsibility. Thera are many other programs that work as well, such as <br />"Imaging Preview" and "Imaging for Windows". <br />Instructions: <br />1. Insert the CD into yom computer. <br />2. Allow time fm the CD to instill. <br />3. Eject the CD from the computer. <br />4. Reinsert the CD inm the computer. <br />5. You are ready to view the Permit Files. <br />NOTE: A program called QuickView Autoruq which runs the QuickView <br />setup program, has also been copied to this CD. If the Autorov program <br />does NOT work, you will need to manually install the QuickView prog[am. <br />Lvstroctiovs for manually installing QuickView are available upon request <br />with the CD or can be viewed a< www.mivina.sbte.co.us. <br />The Permit File document Images can be viewed in the reception area of the DMG office or they can be purchased on a CD or <br />DVD. The CDIDVD pnrcbase frost rnvers the time and labor of making the CD.
