<br />shall be calculated based on available casing diameter information such that the <br />resulting cement plug will extend vertically for the required vertical length inside <br />the intermediate casing. A dry hole marker shall be installed which will have the <br />name of the operator, well number, ]ease number and the legal description welded <br />on the surface. Surface facilities associated with the various wells shall be <br />removed, and the well pad(s) shall be reclaimed according to BLM specifications. <br />In a December 3, 2003, letter, American Soda indicated that the results of an operational <br />evaluation of Well ZO-1 Z led to a determination that this well needed to be de-pressurized and <br />plugged and abandoned in accordance with the Plugging and Abandorunent Plan provided above. <br />A subsequent letter of December 23, 2003, requested a modification of the Permit a allow <br />conversion of the well to a subsidence monitoring well. This conversion would provide for <br />adequate isolation of the injection cavity while establishing a TDR monitoring and temperature <br />survey monitoring location within the preserdly active well field. The following conversion plan <br />will be required by Minor Permit Modification No. 8: <br />Well 20-12 (EPA Permit No. CO 30858-04552) will be converted to monitoring status in <br />a manner that will isolate the mining cavity and allow the well bore of the short string casing <br />above the cavity to be converted to a TDR and temperature monitoring well. The Minor <br />Modification will be made by adding additional language to Part II. Section D. of the Permit, <br />following the 2"d paragraph of original language. The conversion plan for well 20-12 will be as <br />follows: <br />+ A cast iron bridge plug (CIBP) will be set inside each string of casing at approximately <br />the elevation of the bottom of the short string casing. If it is not possible to run a CIBP <br />inside each string of casing due to irregularities, acement basket will be run inside the <br />casing to establish a cement plug at approximately the elevation of the bottom of the short <br />string casing. <br />+ A cement plug will be placed in the short string casing from the cement basket or CIBP to <br />approximately the dissolution surface elevation. A TDR cable will be strapped to a <br />tubing and placed inside the short string casing to the top of the cement plug. Cement <br />will then be circulated down the tubing and back up the annular space between the casing <br />and the tubing. The cement within the tubing will be displaced leaving the tubing open <br />for temperature surveys. <br />+ A cement plug will be placed in the in the long string casing from the CIDP or cement <br />basket to the surface. <br />+ After the placement of the TDR and the cement plugs, a temperature survey will be run to <br />establish the baseline for this location. The initial baseline temperature survey will be run <br />after allowing sufficient time for the cement to cure, but not more than 30 days after <br />cementing is complete. <br />+ The TDR cable in Wel] 20-12 will be monitored on at least a quarterly basis. <br />+ A temperature survey will be run whenever nearby monitoring indicates that upwazd <br />movement of cavity fluid maybe occurring, but at least once every two yeazs. <br />Part I of the Permit will also be modified to modify the status of the Permitted wells as <br />follows: <br />SOB for Minor Modification No. 8 UIC Permit No. C030858-00000 Page 2 of 4 <br />
