Please check one below and remro this form with your payment to DMG. If a CD is requested and the permit is too large for a CD, you will be called. Please allow up to 5 business days from the receipt of [his signed document for your CD or DVD to be available. ~CD @ $5.00 per disk _DVD @ $20.00 per disk Please check ifyou want a copy of the QuickView installation instructions. Computer requirements to use the DMG document imaging CD (or DVD): 1. A program called QuickView will be copied to the CD and must be installed on your computer in order to use a DMG imaging CD the firs[ time. FuNre use of this or other imaging CD's does not require a new installation of the software. 2. The CD or DVD that you purchase must be in the D: drive. A DVD drive wilt read a CD or DVD. 3. The CD will contain files with the .tif extension. There needs to be an image viewing application for .tif files on your computer that will handle single and multi-page documents. On a Windows XP computer, the default viewing programs are called "Windows Picture and Fax Viewer" or "Paint" and they do not work with QuickView. The Imaging Viewer that comes withpre-Windows XP computers does work. DMG has determined that a program called "IrfanView", which can be downloaded from IrfanView.com, does work with QuickView. After downloading "IrfanView", it needs to be configured as yom default .tif viewer DMG does not support or endorse this software and the licensing and registration are the user's responsibility. There ate many other programs that work as well, such as "imaging Preview" and "Imaging for W inflows". Instructfons• L Insert the CD into your computer. 2. Allow time for the CD to instill. 3. Eject the CD from the computer. 4. Reinsert the CD into the computer. 5. You are ready to view the Permit Files. NOTE: A program piled QuickView Autonm, which runs the QuickView setup program, has also been copied to this CD. If the Autorun progmm does NOT work, you will need to manually install the QuickView program. Ins[roctions for manually installing QuickView are available upon request with the CD or can be viewed at www.miuine.state.co.us. The Permit File document images can be viewed io the reception area of the DMG office or they pn be purchased on a CD or DVD. The CD/DVD purchase cost covers the time and labor of making [he CD.
