SECTION IV -Midterm Adequacy Review and Request for Revision <br />Pursuant to Rule 2.08.3, the Division has conducted a midterm review and has found that certain minor modifications <br />aze needed to ensure future compliance with the Colorado Surface Coal Mining Reclamation Act and the Regulations of <br />the Colorado Mined Land Reclamation Board for Coal Mining. <br />L There has been some miscommunication between the Division and the operator regarding recommended paper <br />handling procedures, subsequent to DMG's implementation of document imaging and the transfer of lead <br />permit review responsibilities from the Denver office to the Grand Junction office. The issues appear to have <br />been resolved, but to be certain, the recommended procedures are described here. No specific permit <br />modifications are warranted in response to this item, but please let us know if you disagree or have any <br />questions regarding the following. <br />a) For revision applications, permit transfer applications and similaz applications or requests, please submit two <br />sets to Dan Mathews at the Grand Junction Field Office and one set also addressed to Dan, but sent to the <br />Division's Denver Office. One set of all such applications should also be sent to the BLM Little Snake <br />Resource Area Office. Review corespondence should be handled the same as initial submittals. OSM should <br />not receive copies of initial revision applications or adequacy response submittals; they should be sent one <br />copy of the revision package in final form following final DMG approval. BLM should also be provided a <br />copy of the finalized revision package. Upon conclusion of the review process, Dan Mathews will be <br />responsible for compiling two finalized revision packages, one for insertion in the Grand Junction permit <br />volumes and one for transmittal to Denver for imaging. The second package will be returned [o Dan in Grand <br />Junction after the imaging has been completed. <br />b) Bond release applications should be handled as described above for revisions, except that OSM should be <br />provided a copy of the initial application and adequacy response submittals at the same time as DMG and <br />BLM. <br />c) Please submit one set of the Annual Hydrology Report, Annual Reclamation Report, Annual Revegetation <br />Report, and Annual Wildlife Report, construction certifications (unless submitted as revisions) and Annual P.E. <br />certifications to the Denver Office, and one set of each report or certification to the Grand Junction Office. <br />Note that this differs from the original recommendation, but submittal of such documents to each DMG <br />office will facilitate our review of [he reports. <br />d) Routine quarterly and monthly reports should be submitted to the Denver office for imaging. <br />e) Please call Dan for dvection regarding submittals that do not fit the above categories, or if you have any <br />question regarding the appropriate procedure. <br />2. Tab t is a mine permit summary, and includes a description of initial permitting activities and major permit <br />revisions. Certain additional information is requested to clarify and update this section. Please include <br />reference to approval dates for Permit Revision No. 1 and No. 2, and also please include a summary <br />description of the Permit Revision No. 3 changes and approval date. <br />3. The Tab 6 Table of Contents references Appendix 6-6, 2002 Exploration Program. We do not have a copy of <br />this appendix in either of our copies of the permit application document. Please provide a copy of this <br />appendix, or if the Table of Contents reference is in error, please advise, and provide an appropriately <br />amended Table of Contents for Tab 6. <br />10 <br />
