(Golder) has prepared this letter report for the Cripple Creek & Victor Gold <br />Mining Company (CC&V) summarizing the recent liner repair work completed on the lower <br />geomembrane liner in the Phase IV Valley Leach Facility (VLF) Pregnant Solution Storage Area <br />(PSSA). The liner repair work was required in the PSSA to repair damage from two separate <br />incidents: <br />On January 20, 2003, a Caterpillar D9 wide pad dozer blade damaged the lower 100-mil <br />single sided textured geomembrane in the PSSA, creating puncture and scratch penetration to <br />the geomembrane panels L-351, L-354, and L-355. Ames used a Caterpillar 330 excavator <br />and several laborers on January 20 and 21 to remove LVSCF from the area where the <br />geomembrane damage occurred. This incident was documented by Golder and reported in <br />Weekly Report No. 9. <br />On or around April 7, 2003, a rock rolled into the PSSA, near the riser sump location. The <br />rock created puncture penetration to the lower 100-mil double sided textured geomembrane <br />of panel L-426, on the 2H:1 V PSSA slope, and the lower 100-mil single sided textured <br />geomembrane of panel L-436, in the PSSA bottom. This incident was documented by Golder <br />and reported in Weekly Report No. 13. <br />r1 <br />fr`J <br />The following sections provide details of the liner repair work completed in the PSSA. All liner repair <br />work was completed in accordance with the project technical specifications. All work was observed <br />and documented by the Golder Resident Engineer (Mr. Ron DiDonato), and the Golder Lead <br />Geosynthetics Inspector (Mr. Greg McQuaid). The Golder Engineer-of-Record (Mr. John F. Lupo) <br />was also on-site during this time and inspected the repair work. <br />DOZER DAMAGE REPAIRS <br />The liner damage resulting from the dozer incident consisted of puncture penetration and scratch <br />penetration on panels L355, L354, and L351 (see Figure 1).
