<br />C-81-018: RN-02: B. Pavlik 3 <br />Concerns <br />1) New pg. IV-21 talks about temporary refuse storage area #3 on the haul road <br />between RA #1 and RA #2/3. This was presented to DMG in early 1993 as an <br />emergency storage location (MR #80). DMG did not approve this location. We <br />required additional information for this proposal. Please submit the additional <br />information the Division requested during MR-80 regarding temporary storage of the <br />refuse on the haul road. (Barb, approved page IV-28 has the wording in too.) <br />2) New page IV-22 says the subsoil pile will not be used until reclamation of the final <br />refuse sub-area is started. Does this mean that Western Fuels won't use subsoil piles <br />2/3 and 4 until reclamation of sub-area #11? Will Western Fuels haul the salvaged <br />subsoil all the way across the wash to proposed sub-area #11? (Refer to Map 76) <br />If this is Western Fuels intent nothing more needs to be done to this page. If this <br />is not the long range plan, please reword this portion of the permit to clarify which <br />subsoil piles will be distributed where. <br />3) This may be an issue that will best be dealt with in the revision for construction of <br />Refuse Area SA. Western Fuels-Utah should provide an updated map 82. The <br />current Map 82 does not show topsoil stripping depths for refuse sub-area SA. <br />4) New pg. IV-22 talks about ditches around topsoil and subsoil piles identified on <br />Map 77. Where is Topsoil 2/3, Subsoil 2/3, and Topsoil 4? They are not included <br />on this map. Please revise this map to show actual location of these piles and <br />sediment control measures. <br />5) New page V-34, Table V-7 (old pg. V-39) states that during 1981-1989 520.6 acres <br />were reclaimed at the refuse area. This information is inaccurate. Please update this <br />table to show actual and current reclamation schedule. <br />6) New page V-45 changes the mulch application rate from 1.5 tons of straw per acre <br />to 1.0 ton per acre. Has Western Fuel's had success with this lower mulch rate? <br />Please explain this proposed change in the mulch rate. <br />7) New page V-47 (old pg V-55) discusses a riparian community reference area. Has <br />this reference area been determined? If so, Western Fuel's should reference which <br />map it can be found on. <br />8) Revegetation Success Criteria formula, new pg. V-59: There are problems with <br />Western Fuel's Sample Adequacy formula the way it is written. To start with there <br />are printer errors which made the formula on their submitted page incorrect. <br />Secondly their formula is in disagreement with that the Division accepts for <br />determining sample adequacy.
