, <br />_ . <br />Authorization ID: SAL378 <br />Contact ID: CO MARBLE <br />Expiration Date: 12/31/2012 <br />Use Code: 753 <br />FS-2700-4b (05/03) <br />OMB No. 059GOOS2 <br />U.S. DEPARTMENT OF AGRICULTURE <br />Forest Service <br />FOREST ROAD SPECIAL USE PERMIT <br />AUTHORITY <br />FEDERAL LAND POLICY AND MGMT ACT, AS AMENDED October 21,1976 <br />81201 (hereafter called the holder) is hereby authorized to use and maintain the following <br />described National Forest Development Road within the San Isabel National Forest for the <br />following purposes: <br />Colorado Marble, Inc. is authorized to perform surface replacement and heavy maintenance, in <br />accordance with Exhibit B which is hereby attached to and made a part of this permit, on Forest <br />System Road 228. This Forest Road special use permit is issued in lieu of an approval for road <br />use and maintenance through a mining Plan of Operations (POO). If a POO should be approved <br />in the future this permit will not be necessary. In addition this authorization does not allow for <br />the plowing of snow on this facility. <br />The road covered by this permit is located in the County of Chaffee, State of Colorado, and is <br />shown on the attached map titled Exhibit A and identified asfollows: El/2, Section 27, T.50 N <br />R. 6 E., NMPM. <br />This permit authorizes use of approximately 2.20 miles in length and 60 feet in width or 16.0 <br />acres. <br />THIS PERMIT IS MADE SUBJECT TO THE FOLLOWING TERMS, PROVISIONS, <br />AND CONDITIONS: <br />1. This permit is subject to all existing easements and valid rights existing on this date. <br />2. Holder shall comply with applicable Federal or State law and shall comply with State <br />standards for public health and safety, environmental protection, and siting construction, <br />operation, and maintenance if those standards are more stringent than applicable Federal <br />standards. <br />3. The holder shall cut no timber except as authorized by construction stipulations or <br />maintenance agreements. <br />4. Holder shall pay the United States for all injury, loss, or damage, including fire suppression <br />costs, in accordance with Federal and State laws and regulations. <br />
