Each application shall contain a list of all other licenses and permits needed by the applicant to <br />conduct the proposed surface or underground mining activities. This list shall identify each <br />license and permit by: <br />A list is provided on page 10 Tab 3. <br />Rule 2.03.10(1) Type of permit or license; <br />The application is in compliance with this rule. See page 10 Tab 3. <br />Rule 2.03.10(2) Name and address of issuing authority; <br />With the exception of the address of MSHA, the application is in compliance with this rule. <br />Rule 2.03.10(3) Identification numbers of applications for those permits or licenses, or, if issued, <br />the identification numbers of the permits or licenses; and <br />The application is in compliance with this rule. <br />Rule 2.03.10(4) If a decision has been made, the date of approval or disapproval by each issuing <br />authority. <br />Dates of expiration are provided, but not dates of approval. <br />Rule 2.03.11 Identification of Location of Public Office for Filing of Application. <br />Each application shall identify, by name and address, the public office where the applicant will <br />simultaneously file a copy of the application for public inspection under 2.07.3(4). <br />Applications are placed on file at the Division and the Hayden Public Library (page 11 Tab 3). <br />However, "compliance records" are placed on file at the office of Hydro-Environmental <br />Solutions, Inc. This information is lacking. <br />Rule 2.04.3 General Requirements: Site Description and Land Use Information. <br />Rule 2.04.3(1) Each permit application shall include a description of the existing, premising <br />environmental resources within the proposed permit area and adjacent areas that may be <br />affected or impacted by the proposed surface or underground mining activities. <br />The application is in compliance with this rule. <br />Rule 2.04.3(2) The application shall contain a statement of the condition, capability, and <br />productivity of the land within the proposed permit area, except for underground mining which <br />Page 11 of 38 pages, November 8, 2005 <br />
