The steps <br />were: <br />1. Notify those residences, per Rule 4.08.3(1)(b), which had not been notified <br />previously, of the blasting schedule. <br />2. Notify those persons, per Rule 4.08.4(3), who had not been notified previously, <br />of the meaning of the basting signals. <br />3. Submit a complete minor revision which includes a map showing residences <br />within one-half mile of the permit area, and a list of names and addresses of <br />owners and occupants of those residences. <br />With regard to item 1 and 2, enclosed is a copy of the letters which were sent to thirty <br />recipients. I decided to send the letter to all interested parties which includes <br />residences which had not been notified previously of the blasting schedule and the <br />meaning of the blasting signals. In lieu of sending the letters out certified, enclosed is <br />a certificate of service which certifies that a letter was mailed to each recipient on the <br />list. <br />With regard to item 3, enclosed is a list of adjacent and nearby structure owners and <br />residents along with a 8Y" X 11" Surface Ownership Key Map. If the DMG wants to <br />include this list and key map in the permit, I recommend it be placed in Volume III, <br />Exhibit 2 or 14. I do not think it needs to be incorporated into the permit document. <br />
