Lochhead <br />ERec wive Duecbr <br />Michael B Long <br />Drv~sion Dneno. <br />On June 27, 1994, the Division of Minerals and Geology received your <br />110(2) Reclamation Permit Amendment application (AM-002) for the Golden Wonder <br />Mine, Permit No. M-78-091UG. Preliminary review determined that the following <br />items must be submitted before technical review of your application can begin: <br />EXHIBIT I - Proof of Filing with County Clerk: You must provide an affidavit <br />or receipt indicating the date on which the amendment application was placed <br />with the Hinsdale County Clerk and Recorder for public review. <br />EXHIBIT J - Proof of Mailing Notices of Permit Aoplication: You must submit <br />proof of mailings of the "Notice of Permit Application" (forms were contained <br />in the application package) sent to the Hinsdale County Board of County <br />Commissioners and board of supervisors of the local soil conservation <br />district. We suggest you submit either a date-stamped receipt for certified <br />mailing or a date-stamped copy of the notice form with signature <br />acknowledging personal service and receipt by the appropriate board. <br />You may amend your application by submitting the requested items. In accordance <br />with Rule 1.5, a new decision date will be established 30 days from the date of <br />receipt of all requested items. If you have already initiated publication, you <br />will need to republish notice which specifies a 10 day public comment period from <br />the new date of newspaper publication. <br />This letter shall not be construed to mean that there are no other technical <br />deficiencies in your application.
