Section II of <br />the review document contains a description of the required <br />revisions. <br />RESPONSES TO REQUIRED REVISIONS <br />1. The first page of the application sheet has been changed to <br />reflect the recent change of address for Salt Creek. <br />2. The Identification of Interests section of the permit has <br />been revised. <br />3. An cover page has been added to Appendix D explaining that <br />the Certificate of Insurance is renewed annually and that the <br />Insurer has comitted to notify the Division of any changes <br />such as failure to renew. <br />Salt Creek has also reviewed the Division's recalculation of the <br />reclamation costs. Since a request to require revision of the <br />costs was not contained in Section II of the Review document, Salt <br />Creek was not sure how the Division wanted this item addressed. <br />After discussing this item with you on Thursday last week, it was <br />apparent that the Division expects the reclamation cost <br />recalculation estimate to be included with the Mid-Term review. Due <br />to the complexity of the calculations and the lack of time <br />and staff to review them, Salt Creek request an extension to respond <br />to that item until April 13, 1987. <br />Zt is assumed that the responses should be in the form of a <br />technical revision to the permit.
