An application for a new permit was made on <br />November 7, 1988. The Department of Health has "already" issued <br />a new discharge permit to Grand Mesa. The new permit has been <br />converted from an individual to general status because reclama- <br />tion of the mine site has been completed. The new permit is ef- <br />fective from December 14, 1988 through June 30, 1992. <br />Enclosed is an application for a minor revision to incorporate <br />the new discharge permit into Grand Mesa's mining permit. The <br />new discharge permit, Permit No. COG-400000, Facility No. COG- <br />400006, is to be inserted in Volume 2 Figure 2.04.7(2)(bi-7 of <br />the mining permit.
