Preliminary review determined that the <br />following items must be submitted before technical review of your application can <br />begin: <br />A~pl.ication Signature Page: The four copies of the application form that you <br />submitted did not contain copies of the signed signature page. Please submit <br />four copies of that page. <br />EXHIBIT R: Proof of Mailing Notices of Permit Application to the Jefferson <br />Courlty Commissioners and the Jefferson Soil Conservation District. <br />EXHIBIT 5: Proof of Filing with the Jefferson County Clerk and Recorder. <br />Failure to submit the items listed above by the initial decision date of July 26, <br />1994, may result in a decision to deny your application. You may amend your <br />application by submitting the requested information. In accordance with Rule 1.5, <br />a new decision date will be established 90 days from the date of receipt of all <br />requested items. If you have already initiated publication, you will need to <br />republish notice which specifies the final date for receiving comments is the 20th <br />day after the fourth publication or the next regular business day. <br />This letter shall not be construed to mean that there are no other technical <br />deficiencies in your application.
