Preliminary review of the information received <br />determined that the following items must be received before the Division can consider your application as being submitted and <br />technical review can begin: <br />APPr.1CATinN FORM: page I, Item I: The Division requires [hat [he Permittee name be the same as the name on the <br />Pemut. Page 3, Item 14: The Division requires that the contact person be from the same company as [he Pemlittee or <br />someone authorized to submit items on the Pemdttee's behalf. Page 5, Responsibilities as a Permittee: The Division <br />requires that these be initialed by a corporate officer of [he current Permittee. Page 7, Signature Page: The Division <br />requires that the current Pemtittee sign this page in the presence oC a Notary Public. <br />RXNiRiT n - Prnnf of Mailing of Nnfirrc fn Cnup~ Cnmmiccinnerc and Snil Cnnservafinn Dicfrirt (j;nle 6.4.171 <br />Please submit proof of actual delivery or proof of mailing by Certified Mail, return receipt requested, of Notice of the <br />Application to the local Board of County Commissioners. <br />RXHiRIT S -Permanent Man-Made Sfrnchrrec (Role 6A.19L• You must provide information sufficient m <br />demonstrate that the stability of any stmetures located within two hundred (200) feet of the operation or affected land will <br />not be adversely affected. <br />Your application will not be considered submitted until the information listed above is received and found sufficient to begin our <br />review. A decision date will be established 90 days from the date of receipt of all of the requested information. Additionally, if <br />you have already published notice you will need to republish notice, after the Division considers the application submitted. This <br />notice must be published once a week for four (4) consecutive weeks, starting within ten (10) days of the date your application is <br />considered submitted. We will notify you when you should initiate mpublication of your notice.
